2011/2012 School Pre-participation Physicals
July 10, 2011 – 2:51 amPhysicals for the 2011-12 school year will be held Saturday, July 23 from 8 to 11 a.m. at the TriHealth Fitness & Health Pavilion at 6200 Pfeiffer Road on the Sports Arena (first floor).
No appointment is necessary. Download the Ohio state physical form at ohsaa.org. All athletes must wear shorts.
The cost is $20 per athlete. More information follows:

Who: Any student-athlete entering grades 7-12.
Where: TriHealth Fitness & Health Pavilion

Bring: Completed & signed by parent/guardian, Ohio State Physical Form

Wear: All student-athletes must wear shorts.
Cost: $20.00 per athlete. $10.00 of each fee is returned to the student’s school in
the form of a gift card made available through School Health Supply Co. for the
purchase of sports medicine supplies. Check or cash only.

Directions:
From North:
Take I 71 to Pfeiffer Rd. Exit 15.
Turn right at ramp.
Turn into first driveway on the right.
From South:
Take I 71 to Pfeiffer Rd. Exit 15.
Stay in the outer left hand turn lane on
the exit ramp. Turn left and turn into first
driveway on the right past the highway exit.
